In order to ensure your child has a healthy and productive school year, the Department of Health and Human Services works with Stamford Public Schools, as well as some local private and parochial schools, to set requirements for school entry.

 

Physicals

We recommend that all students have a yearly physical, or well-child visit, to make sure they are up-to-date on vaccines and check on their growth. Students are required to have a yearly physical before they enter Kindergarten, 7th grade, and 10th grade.

We recommend that you schedule your child’s physical around their birthday. It can be difficult to get an appointment right before school begins in August. Don’t risk your child not being able to start school on time due to lack of compliance. Please bring this form with you to your appointment. Your doctor will need to fill it out for the school nurse.
